
Health Bus services are available for individuals who often face barriers to accessing health care services, such as those experiencing homelessness or who are under-housed; immigrant, refugee and newcomer communities; LGBT2SQ community members; and people who face barriers connecting with their primary care providers.
Care is provided by a multidisciplinary team that includes Nurse Practitioners, Mental Health Counsellors, Case Managers, Program Workers, Community Support Workers that bring lived experience to their work and other specialized care providers.
We currently provide services under two programs:
Moss Park Interprofessional Primary Care Program
The Moss Park Interprofessional Primary Care (IPC) Program is a collaboration between Sherbourne Health and Street Health to provide low-barrier primary care services to those who are experiencing homelessness or who are underhoused. This program operates only in the early evenings and weekends within the Moss Park area. It is staffed by Case Managers, Nurse Practitioners and Community Support Workers.

Health Bus Specialized Community Program
Health Bus Specialized Community Program stops happen across Toronto.
Services at these stops are geared towards client needs, for instance, Hepatitis C services, diabetes services or health promotion campaigns. Some stops are open to all while others are specifically for clients of a particular agency.
